benefit included in your Blue Cross medical contract and there is no
cost to you for the eye care program, (except for purchased eye care

For members enrolled in the Prudent Buyer Plans (PPO),
please have your medical provider or physician contact the Managed Care
service unit to obtain authorization for hospitalizations and certain
medical procedures.

your Primary Care Physician is the first person you should consult for
medical care. He or she is responsible for providing you with
primary medical care, for determining when you need referral care, and
