Enhance your employees' health
care coverage: by adding a Anthem blue cross Small
Group Dental plan to your company's benefit package. We offer a
variety of coverage and price options, allowing you to provide the dental
care your employees need, within your company's budget. Like the Anthem blue cross medical plans, you may choose one of the following three financial
options to provide dental coverage for your employees:
Defined Contribution 15
Limits your monthly contribution to dental coverage
to no more than $15 per covered employee
Defined Contribution Select
Empowers you to pay any fixed monthly amount in excess
of $15 per employee, or
Traditional Contribution
Enables you to contribute a percentage of 50% or more
of the total employee premium
For all these options, your employees pay any balance
of the premium.
Under Employee Elect Plus, you, the employer, also
decide which dental plan options your employees may choose from.
BC Life & Health Insurance Company offers two plans
that are richer in benefits and access , and are consequently offered
at a relatively higher premium. These plans, available to groups of
three or more, are:
High Option PPO
Standard Option PPO
When you use a dentist from the PPO provider network,
you will benefit from services provided at rates that are substantially
less than the dentist's usual billed fees. You may visit a provider
from outside the network if you choose, but your share of costs may
be higher.
Anthem blue cross also offers three plans featuring lower
premiums and co payments. The Basic Option PPO is available from BC
Life & Health Insurance Company to groups of three or more; the
other plans are available to all small groups, including groups of two
or less: They are

Anthem blue cross Dental Select HMO
All plans have a network of dentists and dental specialists
throughout California, collectively providing you and your employees
a full range of dental services at reduced costs negotiated especially
for Anthem blue cross members.
Ultimately, the dental program or plan and employer
premium contribution you choose combined with Anthem blue cross' negotiated
provider discounts can significantly reduce out-of-pocket costs providing
an incomparable value for your premium dollar.
Specific Anthem blue cross dental plan benefits, access to
out-of-network providers and monthly premiums vary by the level of coverage
you choose and by plan type. All plans provide preventive care, diagnostic
services and minor and major restorative procedures. Some plans also
feature orthodontic care.
The Anthem blue cross Dental Select HMO and Dental Net plans offer complete care by an exclusive group of Dental HMO providers. When
you choose one of these plans, you may only visit a plan provider for
your dental care. No benefits are avail-able from providers outside
the network.
If your Small Group employees are located in an area
outside of Anthem blue cross' extensive network, Fee for Service (FFS) plans
will be substituted for PPO dental coverage. FFS plans allow your employees
to select a dental provider in their area. Anthem blue cross will reimburse
the provider based on the billed charges, not to exceed the maximum
allowable fee. The maximum allowable fee is determined by Anthem blue cross.
Anthem blue cross of California also offers Voluntary Dental
Plans for small business groups. Two plan options are available - PPO
Dental and Dental Select HMO - to groups that meet the minimum participation
requirements. Voluntary Dental is intended to be 100% employee paid,
therefore, there are no employer contribution requirements, and offers
a meaningful coverage alternative in situations where Small Group Dental
coverage is not extended. Note that Voluntary Dental Plans cannot be
combined with Small Group Dental Plans within a group.

involve features, cost and access to providers. When choosing your dental
plan or plans, consider the features you value most and then select
the plan or plans with those benefits. Plan features are summarized

of Features prepared especially for each plan.
